Influence of Seat Belts in Reducing Traumatic Injuries and Fatalities in Motor Vehicle Accidents | Part 2

The use of lap-shoulder belts has decreased the severity of spinal cord injury in frontal impact collisions without head injury. Several features of car seats have been shown to affect the degree of injury in rear impact collisions. One is a stiffer seat back, which allows less head and neck acceleration and therefore less shear stress. The presence of a headrest at the level of the ears will limit the degree of cervical extension. A shoulder strap will decrease the movement of the chest and thus decrease the forces on the cervical spine also.

Even though restraint systems have been shown to be very effective in preventing injury, they also present the potential for injury.

Air bags have been associated with injuries to the head, neck and upper extremities. A postulated mechanism of injury is the force involved necessary for deployment of the air bag. In one air bag design, deployment of the flap occurs at 360kph.

Neck injuries are usually minor; however, cervical strain injuries can be seen with air bag deployment. Lower extremity injuries have not been reported. Upper extremity injuries have been documented, usually involving the forearm, wrist, and hand. Specifically, these injuries have included lacerations, fracture, dislocations, and extremity flailing. Fatalities also have been associated with air bag deployment.

Overall, air bags and seat belts are a safe technology, and injuries related to these restraints are relatively infrequent and minor. Most of the available data pertain to the driver side air bags, and more study is needed with passenger side air bags.
